Social Worker, Social Pedagogue for the Specialist Service Social Health Care

We are looking for you as soon as possible as a Social Worker, Social Pedagogue (m/f/d) in part-time 34 hours per week for the Health Department – permanent. The Health Department is a public institution that takes care of the protection and promotion of the population's health. Preventive health protection for Dortmund citizens is very important to us; the health awareness of the individual should be positively influenced. Tasks include health counseling, monitoring of hygiene regulations, health care and prevention. The area of responsibility includes substitution treatment within the methadone outpatient clinic, psychiatric treatment of patients of our mobile medical service with consultation hours for homeless, uninsured and precariously living people, as well as counseling, care, diagnostics and home visits for alcoholics, drug addicts and mentally ill people including assessments according to PsychKG and care law.

You advise and support clients in consultation hours and especially in the context of independent home visits. You advise the social environment and collect social histories and, if necessary, social reports, for example to initiate legal guardianship or a rehabilitation measure. You mediate and coordinate necessary assistance within the framework of individual assistance planning and work closely with psychosocial services, institutions and service providers. You take over the social counseling, care and referral of patients of the medical consultation hours for homeless people (MMD) and also perform outreach work and streetwork. In crisis situations you carry out suitable crisis intervention measures and assist with placement in a psychiatric hospital. You take on the administrative tasks associated with case processing and document your work safely and reliably with common PC applications.

You have a completed (technical) university degree in Social Work with state recognition. You bring relevant professional knowledge and experience in outpatient or inpatient residential assistance, psychiatric clinics, socio-psychiatric service or homeless assistance. You are willing to continuously deepen your professional knowledge and skills through appropriate further training. You have skill and flexibility in dealing with different social situations and can safely assess risks and act appropriately even in difficult situations. You deal professionally and prudently with mentally ill and addicted people as well as challenging behaviors and have a pronounced awareness of your own safety. You are willing to perform sovereign tasks, apply liberty-restricting measures and participate in team supervision. You are flexible and mobile. Since the activity is partly outreach, the use of a private motor vehicle for official trips, e.g. home visits, is desirable.
