Registered Nurse Homecare Düsseldorf, Duisburg, Wuppertal

B. Braun Gesundheitsservice GmbH is looking, as soon as possible, for a registered nurse (w/m/d) for patient management in HomeCare sales. Care area: Düsseldorf - Duisburg - Wuppertal. The position is permanent with 40 hours per week.

• Coordinate patient transfer from inpatient care, in contact with discharge management, inpatient units and practising doctors
• Ensure home usage advice and care for patients, order medicines and aids on time, coordinate with health insurers and pharmacies
• Regular follow-up checks and documentation using an iPad
• Act as a networker between patients, clinics, doctors and care facilities and proactively support sales measures

• Completed 3-year training as a registered nurse (w/m/d) or as a Pflegefachfrau / Pflegefachmann (w/m/d)
• Experience in homecare and further training as a care expert (stoma / continence / wound / IV therapy) or ICW wound expert desirable, but not required
• Class B driving licence absolutely required
• Willingness to move to the care region, or living in the region to be covered
