Registered Nurse Homecare Care, Cologne - Düsseldorf - Solingen Region

B. Braun Gesundheitsservice GmbH is looking for a registered nurse (f/m/d) for patient management in HomeCare (IV therapy, enteral nutrition, stoma, continence and wound care), starting as soon as possible. Care area: Cologne - Düsseldorf - Solingen. The position is permanent and 40 hours per week.

• Coordinate patient transfer from inpatient care in constant exchange with discharge management, inpatient units and resident doctors (f/m/d)
• Ensure home application advice and care for patients (f/m/d), order medicines and aids on time and coordinate with health insurers and pharmacies
• Regular progress checks and documentation via iPad for optimal care
• Act as a "networker (f/m/d)" linking patients / customers, clinics, resident doctors (f/m/d) and care facilities, and proactively support sales activities

• Completed 3-year training as a registered nurse (f/m/d) or as a qualified nursing professional (f/m/d)
• Experience in homecare and further training as a nursing expert (f/m/d) in stoma / continence / wound / IV therapy or as a wound expert ICW (f/m/d) is desirable but not required
• Class B driving licence is absolutely required (for the practical part of onboarding in the care area)
• Willingness to relocate to the care region is required, or you already live in the region to be served
