Organization and management of patient care in senior facilities, clinics, or nursing services in Bremen and the surrounding area. Patient admission, data recording, and creation of patient master records. Consulting, provision, and instruction on complex medical aids such as stoma, parenteral and enteral nutrition, wound care, tracheostomy, and incontinence. Creation of therapy plans and order and prescription management. Preparation of prescriptions and execution of consultation documentation. Active networking and outreach to care facilities.

Consulting and provision of medical aids (stoma, nutrition, wound care, tracheostomy, incontinence). Creation of therapy plans, order and prescription management. Preparation of prescriptions, consultation documentation. Active networking and outreach to care facilities.

Completed training as a health and nursing professional or geriatric nurse, ideally with further training as an ICW wound manager. Experience in HomeCare field service desirable. Enthusiasm for individual medical aid provision. Strong service orientation and openness. Class B driver's license.
