Patient Information Center Staff Member

UKSH seeks a staff member for its Patient Information Center in Lübeck to start as soon as possible. Individual advice and training help families move from hospital care to care at home. A 24-hour weekly portion of the role is permanent; the remaining hours initially run until 30 June 2027.

• Contact patients and their relatives, and provide advice and training on arranging care at home through the family care program
• Provide individual practical care training for patients and relatives, both during hospital stays and at home after discharge
• Advise on advance healthcare directives and guardianship law, give talks and represent the service
• Work across professions within UKSH and with external partners, including dementia specialists at LOGGiA, care support centers and other networks
• Support billing and quality assurance for funding bodies and within UKSH

• Completed three-year vocational training as a registered nurse and initial experience in home nursing and social welfare law
• Further professional training or many years of experience in instruction, education and advice, or further training in case management
• A high level of independence, creativity and focus on solutions that account for each family's circumstances
• Confident use of MS Office, including Word and Excel, and Agfa ORBIS
• A category B driving license
