Social Worker Social Counseling / Discharge Management

• Counseling and support of inpatient and outpatient patients within the structured discharge management
• Support of patients and relatives in social and psychosocial crisis situations
• Counseling on individual benefit entitlements and social law issues taking into account applicable social legislation
• Coordination and referral of appropriate support and aftercare services
• Network work and interdisciplinary cooperation with all involved disciplines, cost carriers and relevant committees

• Completed degree in Social Work/Social Pedagogy
• Holistic action competence
• Confident appearance, empathy and a demanding approach to our patients and their relatives
• Interface management
• High self-reflection and ability for self- and organization management
• Teamwork, flexibility and willingness to cooperate as well as legally required proof of measles immunity
